This position will require driving between multiple sites over a defined service area in central Kentucky.

Shift

This position must be willing to work in different environments (both office-based and on the ground), at times best suited to the community (including some evenings and weekends). This includes hours outside of traditional office hours (i.e., 8:00 a.m. – 5:00 p.m. or 9:00 a.m. – 6:00 p.m.)

Job Summary

This position will require travel in a defined service area in the Commonwealth of Kentucky.
The Central Kentucky Health Education Manager will supervise, train, evaluate and support a regional team consisting of 15-20 field staff comprised of Health Education Coordinators ( HEC), Health Education Coordinator Associates ( HEC-A), and/or Peer Prevention Specialists ( PPS) providing regional services in harm reduction programs ( HRP), Syringe Services Programs ( SSP), community based organizations (CBOs) and through targeted outreach activities throughout central Kentucky. This position will serve as the first contact for the HEC, HEC-A, and/or PPS to provide individual consultations to discuss difficult clients/cases, as needed. This position assists in coordinating regional coverage of HIV testing, health education and risk reduction counseling services in concert with local health department SSP and targeted outreach hours and requested testing within collaborating agencies targeting at-risk populations in the community. This position will ensure that the HEC, HEC-A, and/or PPS attend mandatory continuing education programs and trainings related to required interventions and strategies, and will conduct annual personnel evaluations for the HEC, HEC-A, and/or PPS . The Health Education Manager will maintain appropriate program documentation on unique program services, report on regional program statistics, and regularly evaluate case notes and data entry of HEC, HEC-A, and PPS. This position will provide additional training on quality as needed. Finally, this position will assist the Target4 Program Director and Assistant Director in preparing performance reports and narrative as needed, and will assist in data collection/entry into reporting database and attend trainings as necessary.
The Central Kentucky Health Education Manager will serve as a field representative for the Target4 Project administrative team. This position will pilot and review proposed programmatic changes with field staff and advise on statewide implementation strategies. This position will serve as a reviewer and advisor for reporting requirements and assist in integration and implementation to field staff across the state of Kentucky. This position will review staff and patient education materials from a staff/field perspective and ensure tools are practical for implementation and distribution. The Central Kentucky Health Education Manager will serve as a liaison for the Target4 Project and collaborate with local health departments, community-based organizations, and AIDS Services Organizations to integrate HIV testing, counseling, and education into program services. This position will represent the Target4 Project in local health department and community-based initiatives throughout the central Kentucky region. This position will participate in strategic planning and will use Health Department messaging and communications tools (as appropriate). This position must be sensitive to local culture, institutions, and protocols.
This position will provide Ryan White HIV/ AIDS Program Early Intervention Services including targeted HIV and Hepatitis C testing and counseling to persons presenting for services at harm reduction programs ( HRP), syringe services program ( SSP), and through targeted outreach events designed to help the unaware learn of their HIV status and receive referral to HIV care and treatment services if found to be HIV infected. This position will oversee the coordination of services with CDC prevention specialists and linkage navigators throughout central Kentucky. This position will link persons found to be positive for HIV or Hepatitis C to medical care and treatment services at key points of entry for Outpatient/Ambulatory Health Services, Medical Case Management, Mental Health and Substance Abuse Services.
This position will provide Ryan White HIV/ AIDS Program education and risk reduction counseling to persons at high-risk for contracting or transmitting HIV. The Central Kentucky Health Education Manager will assess individual risk and connect them to appropriate resources as needed. This position will provide information about medical and psychosocial support services and counseling to improve the client's health status on educational topics and risk reduction strategies to reduce transmission including comprehensive HIV, Hepatitis C, and STI prevention counseling. This Central Kentucky Health Education Manager will provide information related to pre-exposure prophylaxis (PrEP) for clients' partners and health care coverage options; promote treatment as prevention and harm reduction strategies; as well as connect and refer to medical services as appropriate. The Health Education Manager will provide health education counseling to clients to improve their health literacy, increase retention in medical care, and improve treatment adherence to achieve better health outcomes. This position will distribute patient education materials (pamphlets and books) as well as risk reduction supplies (sharps containers, condoms, lubricant, etc) to persons at high risk for contracting or transmitting HIV. The Health Education Manager may facilitate support groups as indicated by needs of the population served, and attend and actively participate in mandatory continuing education programs and trainings related to required interventions and strategies.
